Databases with PHP

I am new to using PHP to support forms and writing information to Databases. Does Fhih have a group of pre-written scripts that can be modified for use by different users?

Is there a place you can reference which may assist me?

I think we might have a form script on fantastico on server 1, but I'm not sure.

we don't have any scripts. Only users on server one have fantastico, but all others can try hotscripts.com or download the software they want (for example a forum from phpbb.com)
