I need a complex php/flash form... 250FRIH$

Hi all,

I need a complex php form for one of my websites. It needs to submit the contents to an email address I will specify in a PM if you're interested in doing this for me. The form has to show the following:

Name: <text input>
Email Address: <text input>
Type of Enquiry: <drop-down box>

The "Type of Enquiry" drop-down box should have the following options:
  • General Enquiry
  • Request a Quote
  • Report Abuse


When the user selects "General Enquiry", there should be a plain input text box with multiple lines.


When the user selects "Request a Quote", there should be the following fields available:
Your Organisation Type <drop down with following options>
  • Personal
  • Charity / Not-For-Profit Organisation
  • Business

Requirement 1 <text input field (one line)>
Requirement 2 <text input field (one line)>
Requirement 3 <text input field (one line)>
Requirement 4 <text input field (one line)>

Next to the "Requirement 4" text box, there should be text saying "If unsure, type '?'"

Other Comments / Requirements <multi-line text input field>


When the user selects "Report Abuse", there should be the following fields available:
Site Causing Concern <single line text input field>
Details of Concern <multiple line text input field>


Please let me know if this is possible. I'd prefer this in php, but if it is only possible using Flash then I would implement that. I may need help putting the flash code into my html if necessary.

I'm quite flexible in the pricing - if you think this would take a long time to code and is worth more than 250FRIH$ to you, please let me know. However, this is the ballpark I am setting.

I am not a php scripting kind of guy, but have you tried phpformgenerator at http://phpformgen.sourceforge.net/

These parts will require javascript if not using flash. Either in the form of using separate javascript or AJAX.
I don't do javascript, though I would be willing to do just the php part for $190 and I would have a try at the javascript for the rest of the money, though I can't guarantee I would be able to do that.

How much validation would you like done. For instance, if I submit an in-valid email such as 'matt' (no '@' or domain), would you like to return to the form and ask them to sort it out, or just send you the in-valid email?

If we're talking FRIH$, you may have a deal Wink I honestly had / have no idea how long this would take or how much effort is involved, so payment terms are pretty much entirely up to you. Just tell me what you want.

I would say that as long as there is some text entered that contains @, this would be fine. The only mandatory fields in the form would need to be Name, Email Address and Type of Enquiry.

If this sounds ok, let me know and I'll PM you the details.

I would expect it take a couple of days, maximum, and then another day maybe for any changes you'd like (depending on what they are).
If the time-scale is good for you, I'd actually rather like to do this Smile

I asked for a quote at DDS - still not heard anything back. I'd be surprised if you guys started to make it without giving me an idea of how much you were planning to charge etc.

Hi , I can code this. I'll use Ajax , Javascript for the client side form
and submission with PHP. But that would be compatabality issues.

Thanks for all the offers of help, but mathiaus has created the perfect solution for me (custom designed php with javascript - it's most impressive!) Very Happy
