email form handling on a windows 2000 server help.

So, I have a question regarding email forms. At my work they are running a windows server 2000 box with IIS 5 and I need to create form that emails the results back to my boss in a format that looks the same as the input form on the webpage. I've tried a little ASP script which uses some CDO (collaboration data object) stuff. but the either the server or the exchange server (hosted on a different box) isn't configured right, or somethings wrong with my code which I used from a tutioral I found on the web.

So my question is, does anybody know how to return the results of an email form in a format that is formated to look like the form people enter there info into? what is the best way?
