Accident occurred Monday, December 23, 1991 in RAINBOW LAKE, FL
Probable Cause Approval Date: 5/5/1993
Aircraft: PIPER PA-34-200T, registration: N47506
Injuries: 2 Fatal.
THE PRIVATE PILOT AND A PILOT RATED PASSENGER WERE GOING TO PRACTICE SIMULATED INSTRUMENT FLIGHT. WITNESSES OBSERVED THE AIRPLANE'S RIGHT WING FAIL IN A DIVE AND CRASH. EXAMINATION OF THE WRECKAGE AND BODIES REVEALED THAT BOTH OCCUPANTS WERE PARTIALLY CLOTHED AND THE FRONT RIGHT SEAT WAS IN THE FULL AFT RECLINING POSITION. NEITHER BODY SHOWED EVIDENCE OF SEATBELTS OR SHOULDER HARNESSES BEING WORN. EXAMINATION OF THE INDIVIDUALS' CLOTHING REVEALED NO EVIDENCE OF RIPPING OR DISTRESS TO THE ZIPPERS AND BELTS.

The National Transportation Safety Board determines the probable cause(s) of this accident as follows:

THE PILOT IN COMMAND'S IMPROPER INFLIGHT DECISION TO DIVERT HER ATTENTION TO OTHER ACTIVITIES NOT RELATED TO THE CONDUCT OF THE FLIGHT. CONTRIBUTING TO THE ACCIDENT WAS THE EXCEEDING OF THE DESIGN LIMITS OF THE AIRPLANE LEADING TO A WING FAILURE.

So... the pilot and co-pilot decided to start messing around WHILE FLYING THE PLANE. At least it wasn't a commercial flight. Some people are just idiots Razz

An old news story but funny nontheless.

I want to laugh at this, but it isn't really funny... As a pilot in training, I'm not surprised and bet that this activity is not as uncommon as you would like to believe. Even without autopilot, a properly trimmed plane pretty much flies itself. The mile high club is not restricted to airlines, you know.

but here let's think about this for a minute; even with the best airplanes and technology you still have the human element; it only takes one dummy to cause a major problem -- in this case 2 of them. Hmmm... Sounds like they were very distracted.

Bottom line; even though it's bad that this happened at least they didn't take out a bunch of innocent people on the ground or cause a commercial jet-liner(s) to crash. The thing is, though, is that they could've. Maybe the NTSB should review the status of these flight instructors and impose stronger standards. Maybe, that is so, but then again, maybe not... there will always be the human element and as long as there is, there will always be mistakes... Mistakes, indeed.
