No problem.
But one quick question...do you want it to e-mail the form results to you, store them in a database, or both? I'll get working on what I can (although my time on the computer is limited) without the info. But the sooner, the better, because after my therapy appointment tommorrow (GMT-5) I may not be able to get on much.
