I am either looking for a good free script or a howto to create a custom form that a person can fill out that forwards the information to an email.

If you have Frontpage it is going to be very easy to create a form just make your fields like you want it and then just put a little script in it to email them to you. When I find the script again on my computer, I will post it here.
