Recommend Form - Need Help!

Hello people,
I wanted to make a recommend form for my forum.. By which the form automatically sends a pre - set mail to the recommened person.
The form must be something like this.

Your Name : [Name] Your Email Adress :[address]

People who you want to recommend

Friends#1 : again the same fields name and adress
... continuing upto 10 friends...

and one more thing
In the mail i want it to say Dear [Friend#1's Name], and so on...

can anyone tell me where can i get such a form, or if anyone is good at it. Can someone make me one? Where i can define the pre set Email

You might need to explore the php mail() function as I think this is not possible with mailto forms.

i saw the cgi type form somewhere .. w8 i will search and link it here. i want it something of that sort only.
