I'm gathering that your hosted on Server 2 which has DirectAdmin:
if you haven't already made a database then login to directadmin and click SQl management. then click "create new database" and fill out the required information. Its advised to keep the database username and database name the same. For example if I was to create a database for a forum I would use the similar instences:

Database Name: username_"forum" Database Username: username_"forum" Username Password: password to directadmin Confirm Password: repeat password to directadmin |
Note that the above " " should not be there!
You can now login to www.yousitename.frih.net/phpmyadmin to control the database.
