i create a form and when i submit the details it comes up with this and doesnt work, can anyone help?

go check the errorlog and tell us what it says

also post the code of the script producing the error. And if it mension error in line nr X then post the lines before and that line of code.
If your code worked before then check PHP versions. Maybe your command is not supported annymore.
