To put in to your website, it's probably best to write a completely custom one
Otherwise, why not use something along the lines of Wordpress or a CMS? Those all have features which allow the users to leave comments ^^
you will need php with MySql database or a flatfile.
there are many such free open source available out there. search in the google and u will have what u wanted with a little modifications if needed be.
good luck. its pretty simple. go through the documentation of the program carefully and u will have comment system working efficiently within minutes.
