Please help me to know how they proceed to send me that kind of mail.
Since 2 weeks, another database hosted by them(I supposed is being filled with spam records)
Please help me rapidly to understand how they do and how to block them. Actually, I think you are not hacked, you are spamed. Spamers get your form and they know what thay must fill in. They are using some spam app. for fill your form. It is not easy to say is it something wrong with your send mail script but try to use Captcha or some other spam protection.

You could also try to validate some of the data. For instance, Make sure the area code is within 100 miles of the postal code, and also matches with the town/state. A captcha would also work, but it would be best to use it in conjunction with the previously mentioned techniques.
If they're stupid they may also be spamming from the same place each time. If you look through your logs and see if there's a specific IP or IP range that these messages are coming from, you can block access.
Best way to avoid this is try to validate some filels. i faced the same issue in my phpBB forum. i created some custom profile fields. now its spam free

I noticed that the IPs are not the same, it means that they are using different machines. How can you explain it?
And the date is supposed to be the date of the day but I do't know how they do to change it before doing the insertion.jst gt a good anti virus ad clean up your system.

The real matter is not my machine. I am using a shared hosting so I can not install an antivirus on it. I just need a method to fight against spammer that are spamming me.

Without seeing the code, it's not really obvious what's going on. Perhaps you could post a snip of the code that handles the form's post data?Try using reCaptcha?

It's a free service and generally considered the hardest captcha to crack...
